Short answer: If pain, burning, numbness, or tingling runs from your low back through the buttock and down one leg — especially past the knee — you may have sciatica, not “just a pulled muscle.” At Adisi Health Center in Hinsdale, Dr. Steve Petzel evaluates the source of that nerve irritation and builds a stacked care plan that can include chiropractic adjustments, Class IV laser, EMTT, Winback TECAR, and Storz Medical shockwave depending on the cause. Call (630) 887-1900 or schedule an appointment when leg pain is limiting work, sleep, or walking.

What is sciatica, and how is it different from regular back pain?

Sciatica is nerve pain. The sciatic nerve is the longest nerve in the body. It starts in the lower lumbar spine, passes through the buttock and gluteal region, and travels down the back of each leg toward the foot. When a nerve root is compressed or chemically irritated, pain can feel electric, burning, or sharp — and it often follows a clear line down the leg.

Regular back pain usually stays local in the low back or buttocks. Sciatica’s hallmark is radiating symptoms: shooting pain, numbness, tingling, or weakness that travels into the thigh, calf, or foot. Sitting, coughing, or bending forward can make it worse. Night pain that wakes you up is also common when the nerve is inflamed.

If your pain “travels,” treat it as a nerve problem until a proper exam says otherwise. Rest and over-the-counter anti-inflammatories alone rarely fix true nerve compression.

What causes sciatica near Hinsdale?

Several structures can irritate the sciatic nerve. Adisi’s sciatica treatment page lists the most common local causes we evaluate:

  • Herniated or bulging disc — disc material presses on a lumbar nerve root (often at L4–L5 or L5–S1)
  • Spinal stenosis — narrowing of the spinal canal that compresses nerve roots; symptoms often worsen with walking or standing
  • Piriformis syndrome — a tight or spasmed piriformis muscle in the buttock entraps the sciatic nerve (sometimes missed on imaging)
  • Spondylolisthesis — a vertebra slips forward and narrows the nerve canal
  • Postural and muscular imbalances — chronic tightness in hip flexors, hamstrings, or lumbar muscles that loads the spine unevenly
  • Injury or trauma — falls, heavy lifting, car accidents, or work injuries that acutely irritate a disc or joint

Same symptom. Different cause. That is why a one-size-fits-all “back stretch” plan often fails — and why diagnosis comes first at Adisi.

When should I see a chiropractor for sciatica in Hinsdale?

Book an evaluation when:

  • Pain, burning, or tingling runs from the low back into the buttock and down the leg
  • Symptoms last more than a few days, or keep returning after rest
  • Sitting, driving, or sleeping makes the leg worse
  • You notice mild weakness, foot “drag,” or trouble climbing stairs
  • Pain started after lifting, a fall, a car accident, or a work injury
  • You want a non-surgical plan before injections or long medication use become the default

Early care matters. Waiting until you cannot walk comfortably often means more inflammation, more guarding, and a longer recovery.

When should I go to the ER instead?

Chiropractic care is for stable musculoskeletal and nerve-irritation problems — not emergencies. Go to the ER or call 911 first if you have:

  • Sudden, severe weakness or inability to walk
  • Loss of bowel or bladder control after back or leg pain starts
  • Numbness in the saddle (groin/inner thigh) region
  • Progressive neurological loss in both legs
  • Fever with severe back pain
  • Trauma with suspected fracture, heavy bleeding, or head injury
  • Chest pain or shortness of breath

Once you are medically cleared, we can evaluate the soft-tissue and spinal side of recovery.

How does Adisi Health treat sciatica without surgery?

Dr. Petzel treats every patient personally. Care starts with a history, orthopedic and neurological exam, and spinal analysis (imaging when appropriate) to identify the compression source — disc, stenosis, piriformis, or other.

Then therapies are stacked in one visit when they fit the tissue involved. Our sciatica care page describes the coordinated approach:

  1. Chiropractic adjustments and nerve decompression — restore segmental motion and reduce mechanical pressure on irritated nerve roots. Chiropractic is in-network with BCBS PPO for many patients.
  2. LightForce Class IV laser (40W XLi) — deep photobiomodulation aimed at calming neurogenic inflammation around irritated nerve tissue; sessions are typically brief and comfortable.
  3. Storz Magnetolith EMTT — high-energy magnetic pulse therapy that reaches deep lumbar structures without skin contact; useful for chronic or stubborn nerve irritation patterns.
  4. Winback TECAR — capacitive and resistive energy transfer that generates deep endogenous heat; especially helpful when piriformis tightness or deep gluteal guarding is compressing the nerve.
  5. Storz Medical radial and focused shockwave — targets myofascial adhesions, scar tissue, and trigger points in the gluteal and lumbar region when soft-tissue compression is part of the picture. Piriformis-driven sciatica often responds especially well to TECAR and shockwave together.

Advanced modalities (laser, EMTT, TECAR, shockwave) are cash-pay. Many patients compare that cost favorably with repeated specialist co-pays — and prefer the speed of a stacked visit. For new patients exploring shockwave as part of care, Adisi’s site lists a new-patient special of $249 (consult + first combined session), single sessions at $200, and a 6-pack at $1,050.

Sciatica from a car accident or work injury?

Radiating leg pain after a crash or on-the-job lift is common. Soft-tissue and disc irritation can show up hours or days later. Adisi handles personal injury and workers’ compensation cases, documents findings clearly for insurers, and coordinates care when an attorney is involved. Auto and workers’ comp rules differ — tell us which path applies when you call.

What happens at the first sciatica visit?

  1. History — when it started, what makes it worse, prior imaging or injections, work or sport demands
  2. Exam — orthopedic and neurological testing, straight-leg raise and related signs, gait and range of motion
  3. Clear explanation — what is likely compressing or irritating the nerve, and what is not an emergency
  4. Care plan — which combination of adjustment, laser, EMTT, TECAR, and/or shockwave fits your cause
  5. Treatment begins — often the same day, with home guidance for sitting, sleep, and activity

You see Dr. Petzel every visit — no rotating providers.

Frequently asked questions

How is sciatica different from regular back pain?
Sciatica is nerve pain with radiating symptoms into the buttock and leg. Regular back pain usually stays local. If pain follows a line down the leg — especially past the knee — get a nerve-focused evaluation. Details: sciatica treatment in Hinsdale.

Do I need an MRI before I can start care?
Not always. Many patients start with a thorough clinical exam. Imaging is ordered or reviewed when findings suggest it would change the plan. Bring prior MRI or X-ray reports if you have them.

Can chiropractic help if my MRI shows a disc bulge?
Often yes. Disc bulges and herniations are a common cause of sciatica. Care focuses on decompressing the irritated nerve root, calming inflammation, and restoring function — without surgery when appropriate. Severe or progressive neurological loss still needs urgent medical evaluation first.

What if my problem is piriformis syndrome, not a disc?
Then the compression site is in the gluteal region, not the lumbar disc. Piriformis-driven sciatica often responds especially well to TECAR and shockwave aimed at the deep gluteal muscles, along with spinal care when posture and lumbar mechanics are involved.

Does insurance cover sciatica treatment at Adisi?
Chiropractic adjustments are in-network with BCBS PPO for many patients. Advanced modalities (laser, EMTT, TECAR, shockwave) are cash-pay. We review coverage options at consultation.

How many visits will I need?
It depends on the cause and how long symptoms have been present. Acute cases often move faster than long-standing nerve irritation. We track pain, function, and neurological signs each visit and adjust the plan as you improve.

Can you help after back surgery if my leg still hurts?
Often, with modifications. Post-surgical pain may involve scar tissue or adjacent-segment stress. Laser and EMTT are frequently useful because they do not require mechanical pressure on sensitive surgical sites. Spinal manipulation is only considered after a careful review of your surgical history.

Is this the same as the sciatica service page on your website?
No. The sciatica treatment service page is the full clinical overview of modalities and visit structure. This article answers the patient questions people search first — when to seek care, what causes leg pain, red flags, and how to book in Hinsdale and the western suburbs.
