Plantar fasciitis is one of the most common musculoskeletal conditions treated in outpatient settings — and one of the most frequently undertreated. The name implies inflammation, but in chronic cases, the real problem is tissue degeneration, not active inflammation. This distinction is why anti-inflammatories, ice, and cortisone injections provide temporary relief but rarely produce lasting recovery.

The plantar fascia is a thick band of connective tissue that runs along the bottom of your foot, connecting your heel bone to your toes. It absorbs tremendous force with every step. When it becomes overloaded — through repetitive strain, poor mechanics, inadequate footwear, or sudden changes in activity — microtears develop at the calcaneal insertion point where the fascia meets the heel bone.
